You may schedule an appointment by calling one of our offices directly, or your family physician can schedule an appointment for you. Patients are asked to arrive 30 minutes early for their first appointment to complete necessary paperwork.

Hours: 8:00 a.m.–5:00 p.m. (Some flexibility available)

What to bring:

• Your written prescription
• Nonrestrictive, comfortable clothing and footwear.
  (Shorts are recommended for back or leg problems)
• Insurance information / insurance card
• List of medications with dosages
